Payment-in-Lieu of Transportation Agreement
This agreement is entered into this _____day of __________, 20__, between ____________________ of _______________________, and the Blaine School District. This agreement is for school days beginning ______________, and ending ______________, 20__.
In lieu of providing transportation, the school district agrees to pay a sum of money not to exceed $.505 per mile from the residence of _________________ to/from Blaine School District. Such payment to be based on actual attendance and Mileage Log verification and filed for on the District’s combined claim verification form.
A valid driver’s license and proof of insurance from the transporter must be on file with the District before this agreement can be put into effect.

General Conditions
- The student(s) shall attend the school designated as their attendance center.
- The transporter shall hold the State and/or School District harmless from any incident, occurrence or accident arising out of the transportation of this child to school.
- The monthly payments shall be the daily rate for each vehicle or mile times the number of days transportation was provided, as verified by school attendance records.
- This agreement may be terminated or amended (a) by mutual agreement, (b) when conditions listed above are substantially altered, (c) if the transporter fails to make consistent and adequate effort to transport said child, or (d) if regular or special education pupil transportation service becomes available.
