Unity Care NW’s Mobile Dental Program will be visiting Blaine Middle School and Blaine High School on Tuesday, April 21.
Permission forms are available at each school's office
Completed forms are due by Wednesday, April 15
Students will be out of class for approximately 10 minutes during their appointment
This is a convenient opportunity for students to receive dental care right at school!
